Device Permission Agreement Form

The computer that your child is being issued is an educational tool and should be used in that capacity.
Before a student is assigned technology device for home uses, he/she and his/her parent or guardian must sign the attached permission form.
To assign a device to a student, the parents or guardians must assume financial responsibility for the equipment by signing the Parent or Guardian Device Permission Agreement Form. The appropriate replacement cost of the technology is $300.
If the device has component failure, it will be covered by a warranty repair at no cost to you. If the device issued to your child is lost, stolen or damaged through negligence, or vandalism, the you are responsible for the cost of repair or replacement.
The device issued to the student must be returned to the school at the end of the school year by June 1st of the school year the device was issued in. Failure to return the equipment in a timely manner may result in loss of the student's privilege of taking equipment home again, and may result in a replacement cost payed by the parent or guardian.
All device property of the Lake of the Woods School's District. The Lake of the Woods School's District reserves the right to demand the immediate return of the issued device at any time.
If you, as the parent/guardian, would rather that a device NOT be issued to your child, please inform the school by checking the appropriate box on the permission form and we will honor this request.
The Agreement Form will be filed with your child's school student records.
