Field Trip Bag Lunch Request

    The Lake of the Woods School food service will provide a bag breakfast (if leave time is before our regular breakfast serving time (regular breakfast serving time 7:45-8:10 a.m.) and/or bag lunch for your child to take on his/her field trip. The bag breakfast will consist of a granola bar, juice, fruit, and a milk. The bag lunch will consist of a meat and cheese sandwich, chips, carrots, cookie, fruit and milk.
    There is NO charge for bag breakfast and/or lunch for students. There IS a charge for bag breakfast and/or lunch for adults: (BREAKFAST - Elementary Bag Breakfast = $0.00 - High School Bag Breakfast = $0.00 - Adult Bag Breakfast = $3.00) & (LUNCH - Elementary Bag Lunch = $0.00 - High School Bag Lunch = $0.00 - Adult Bag Lunch = $5.00)
    The parent/guardian is responsible for sending their child’s lunch if they do not request a school lunch. The bus does not stop for food purchases.
    Even if your child decides not to receive the bag school breakfast and/or lunch, please return this form to the teacher.
